What is included in advance directives

If you are unable to communicate your wishes to your medical providers, your family members or designated Health Care Power of Attorney will have to make important decisions on your behalf without knowing how you would prefer for those decisions to be made. By having thought through this issues in advance, and working with your estate planning attorney to draft Advance Directives, you ensure that your medical providers know how to proceed in a way that is consistent with your wishes. 

When we work together to draft your Advance Directives, we will discuss the following:

- Life sustaining measures (artificial respiration, feeding tubes, etc.)

- Duration of life support

- The individual (and successor) to be designated to make these decisions

- Type of care needed (in-home, palliative, hospital transfers, nursing home facility, etc.)

- Amount and types of pain relief

- Funeral and burial instructions

Creating an Advance Directive is an optional and voluntary measure. They can be rewritten or modified at any time so long as you are of 'sound mind'.
